Apply for a disabled person's Freedom Pass

Before you start

You will need the following information to complete your application. 

  • Contact details for your doctor and medical specialists
  • Your National Insurance number
  • Proof of your disability, e.g. a certificate of visual impairment, a letter showing your disability benefit award
  • Two proofs of residence showing you live in Lambeth, e.g. your Council Tax bill, a benefits letter
  • Proof of your identity, e.g. your passport
